What is Patient Connect?

Introducing Patient Connect!

 

For the last couple of months, the team at Polygon Health has worked tirelessly to proudly bring you our first release of the Patient Connect platform.



Patient Connect is a patient network and resource platform aimed at doing one thing: connect you to the right person and/or resource easily and quickly. The platform accomplishes this with just two simple features we'd like to highlight for you today:

 


We match you to other patients based on your story


The first thing you'll see when entering Patient Connect is a prompt to add a pin to the map. When you add a pin, you can enter your zip code and then create your story about your journey, where you can write as little or as much as you want. Afterwards, your pin will be placed on the map based on your zip code, and Patient Connect will automatically analyze your story and find other patients with similar experiences. You can easily browse these stories and learn about patients who may be on a similar journey as you.




Messaging

 

If you discover other people you'd like to communicate with, just simply click the message button on their story profile and a chat will be created. The person on the other side will be notified via email and you're free to take it from there. Our hope is that by matching people with similar experiences, it will greatly help you find the solutions you seek.

 

Of course, we haven't forgotten about the resources. Within the map, there are a number of multiple myeloma care centers, centers of excellence, and nonprofit lodging options that may be of service to you. Additionally, we are building out the resources tab and aim to add to it on further releases. We've heard your feedback on the need for personalized guides and are exploring options to bring that to reality.




What's next?


We hope that these two simple features, patient matching and messaging, will be impactful to you and other multiple myeloma patients around the world. We built this platform to be simple and straight forward so that you can find your solutions as easily as possible.

 

In the future, we hope to build a connected ecosystem to serve not just all multiple myeloma patients but all patients globally.
