Privacy Policy for Patient Connect

Polygon Health (“Polygon Health,” “we,” “our,” or “us”) powers a product called Patient Connect (“App”), a platform that connects patients with each other and with resources to improve support and care.

Please read this Privacy Policy carefully to understand our practices regarding your information and how we handle it. By registering with or using the App, you agree to this policy. If you do not agree with our practices, please do not use the App. This policy may change over time (see the “Changes to Our Privacy Policy” section below). Your continued use of the App after any updates means you accept those changes, so please check the policy periodically for updates.

At Patient Connect, we take your privacy seriously. Below are the key points of how we manage your information:

We Do Not Sell Your Data

We respect your privacy. Your personal data will never be sold to third parties.

Your Information is Kept Confidential

Any personally identifiable information (PII) you share with us is kept private and treated with the highest confidentiality.

Public Areas Are Your Choice

If you post information in public areas of the platform (like chats or community spaces), this is entirely your choice and done at your own risk.

Control Over Your Data

You are in control of your data. You can request to delete, change, or modify your account and personal information at any time.

Your Data is Secure

We take your data’s security seriously. All personal information is encrypted, and we follow strict HIPAA-compliant policies to safeguard your health information.

  • App Updates: Notify you of app updates or changes to the services we offer.

  • For Any Other Purpose: Use your information for any specific purpose you provided it for.

In addition, we use usage data to improve the app and personalize your experience by:

  • Recognizing You: Automatically recognizing you when you log back into the app.

Your Choices About How We Collect, Use, and Share Your Information

We want to give you control over how your data is used. Here are some ways to manage what information we collect and how it’s shared:

  • Cookies and Tracking: You can block cookies or other tracking technologies through your browser settings. However, blocking them may affect how well the app works.

  • Promotions from Patient Connect: If you’d rather not receive promotional emails or messages, you can opt out by emailing us at privacy@patientconnect.com or adjusting your account preferences.

Third-Party Activities

Occasionally, Patient Connect may promote activities like research studies, meetups, or other events run by third parties. Please note:

  • These activities are managed by third parties, not Patient Connect.

  • We are not responsible for any liabilities or issues that may occur during these activities.

Please review the terms and conditions of third-party events before participating.

Sharing Information for Third-Party Promotions

To help third parties (such as event organizers) promote their activities, we may share general, non-private information that doesn’t identify you. Your personally identifiable information (PII) will never be shared without your consent.

Third-Party Services and Integrations

Our app may contain links to third-party services or allow you to log in through third-party accounts (e.g., Facebook, Google). Please be aware that these third parties have their own privacy policies, and we are not responsible for how they handle your data.

Data Retention Policy

We keep your personal information for as long as it’s necessary to provide the services you’ve requested or for other essential purposes such as complying with legal obligations. Once your account is deleted, we will securely remove your data unless we are required to retain it for legal reasons.

Breach Notification

In the event of a data breach, we will notify you within a reasonable timeframe, following applicable legal requirements.
